As long as you haven't had internal cancer, leukemia, or melanoma within the past 5 years – and you haven't had skin cancer in the past 3 years, you can apply for this insurance and receive coverage for any new cancer diagnosed at least 30 days (time limit not applicable in California and Arizona) after your coverage goes into force.
If a cancer diagnosis is made within the first 30 days, this policy will pay 25% of the allowed benefit for the covered service for the life of the policy.

As an adult, you can apply for this insurance regardless of age – whether you’re 19 or 91. (In California and Minnesota, adults under 65 are eligible to apply.)

The Family Plan insures you, your spouse (under age 65 in California and Minnesota), and all unmarried dependent children under the age of 19 (age 21 in Louisiana, 22 in North Dakota, 25 in Texas, New Mexico and Minnesota) – under age 23 (age 24 in Colorado and Louisiana, 25 in Kentucky and Texas, 26 in North Dakota) if a full-time student at an accredited educational institution. (In Georgia, full-time student coverage issued by age 23 can continue through age 25; In Utah, dependent children are eligible for coverage to age 26.)

Your answers to the questions on the application determine your eligibility for this coverage. After we receive your premium and approve your application, you will receive your insurance coverage in the mail. The effective date for your coverage will be indicated on your insurance policy or certificate.

You will never be singled out for a rate increase. Your rate can be increased for other reasons such as an increase applicable to all persons covered under this type of policy in your state. (In Louisiana, premiums cannot be increased during the first 12 months.)
